Application Forms, along with certified copies of supporting documents, should be lodged in person to any Medicare Office or Centrelink Customer Service Centre, or posted to:

Completed applications must include the following certified copies of supporting documents:

Vehicle registration papers;
Proof that you have paid for the parts AND installation of the LPG conversion kit OR proof that you have paid for a new LPG vehicle; AND
Current driver's licence
For completed applications lodged over the counter at any Medicare Office or Centrelink Customer Service Centre, your supporting documents will be copied and certified and the originals returned to you.
For postal applications please DO NOT provide original supporting documents. Certified copies of supporting documents must be provided. Originals will not be returned to you. See the Customer Guidelines for who can certify.

CHANGES TO THE LPG VEHICLE SCHEME:
Program changes apply to LPG conversions or new LPG vehicle purchases completed on or after 1 July 2009.
The following grants are available subject to program eligibility criteria:
1. Grants for LPG conversions of registered vehicles:

Grant Conversion Completed
$1,750 Between 1 July 2009 and 30 June 2010
$1,500 Between 1 July 2010 and 30 June 2011
$1,250 Between 1 July 2011 and 30 June 2012
$1,000 Between 1 July 2012 and 30 June 2013
$1,000 Between 1 July 2013 and 30 June 2014
2. Grants for the purchase of new vehicles fitted with LPG before first registration:
Grant Vehicle Purchase Completed
$2,000 Between 1 July 2009 and 30 June 2014

From 10 June 2009, there is a change to vehicle registration requirements for LPG conversions. A registered vehicle that has been purchased and then converted to LPG prior to registration transfer into the applicant�s name may now be eligible for the grant subject to certain conditions. For more information, call the AusIndustry hotline or read the Customer Guidelines . Customers who consider that they may now meet the eligibility requirements as a result of this change can resubmit their application. Please note that all applications must be lodged within 12 months of the date of conversion or new vehicle purchase.
